Around the world more than 300 million people are missing one or more teeth . Being edentulous is a more common handicap than many realize. Research, documented studies and a long clinical history have proven the effectiveness and long lasting results of dental implants, especially pertaining to the physical benefits of osseointegration and bone preservation .
Many patients seeking a full reconstruction or rehabilitation of a lower or upper jaw are familiar with denture products and wish to avoid them altogether. Some denture products (depending on characteristics of jawbone, amount of gum tissue and quality of denture) can force patients to make changes in eating habits, create new choices of what to eat, use special liners to prevent having loose or painful dentures, have difficulties with speech, etc.
The All-on-4 treatment concept was developed to provide edentulous patients with an efficient and effective restoration using only four implants to support an immediately loaded full-arch prosthesis. The concept has near universal application and has the potential to make this treatment affordable for an even greater segment of our population. Same day implants save a lot of chair time for patients and can eliminate the dread of having to live without a denture. Without exception, patients feel great about themselves after.
The All-On-Four concept refers to a hybrid placement pattern of patented implant devices and fittings that provide the attachment interface of a custom bridge prosthetic. Bridge material can be constructed of acrylic or porcelain. "All-on-4™" for edentulous jaws is a treatment procedure that uses tilting posterior implants. This facilitates an optimal support for an acrylic bridge that can be fabricated and functioning within just a few hours after surgery .
Custom bridgework enables the complete removal of material that commonly causes the gag reflex response when using common dentures. The prosthesis is also known as a palateless denture. The Immediate Function implants allow treatment to be completed in one treatment session, if desired by the patient. Extractions, supplemental grafts, implant placement and periodontal gum suturing can also be completed during the same appointment.
The custom bridge is attached to the implants on a semi-permanent basis. This type of bridge is designed to be removed by the dentist to on a routine basis that assures monitoring of optimal bone and tissue health that is critical for long term servicability of the implant Routine cleaning and maintenance eliminates the common problems associated with inadequate care for implant supported bridgework.

Stability even in minimum bone volume
By tilting the two posterior implants, longer implants can be used in minimum bone volume, increasing bone-to-implant contact and reducing the need for vertical bone augmentation. The tilted posterior implants can be anchored in better quality anterior bone, reducing cantilevers and thus improving support of the prosthesis.
Good clinical results
Biomechanical measurements show that tilted implants, when part of prosthetic support, do not have a negative effect on the load distribution. The tilting of implants has been used in clinical practice for over a decade and has shown good results.
